The download contains one A4 portrait page measuring 595.28 by 841.89 points. It contains six interactive fields: name (120 characters), email (160), phone (40), preferred date (10), message (600), and one consent checkbox. The date is a text field; enter it as YYYY-MM-DD.
A real PDF form, created locally
The browser writes a PDF document with a page object, AcroForm field objects, widget annotations, appearance streams, a cross-reference table, and a trailer. The download uses the application/pdf MIME type. This is a new form template only; it does not parse, edit, fill, or merge an existing PDF.
Private, focused format limits
The title, form creation, preview, and download stay in the current browser. The tool does not upload files, store a draft, collect form values, or submit data.
