Generate PostgreSQL INSERT or UPDATE text from a table name, comma-separated column names and a JSON array of rows. UPDATE requires a non-null unique key and always includes WHERE. Identifiers and scalar literals are escaped, never executed. Limits: 500 KB, 1,000 rows, 100 columns, 20,000 cells; nested values are rejected.
